Social Service Vision
To respond to the growing needs of our Older Adult Community, and to advocate on their behalf as the population continues to grow.
Information and referral to older adult services
Care management and assessment
Support group for diabetes, chronic illness, transitions and caregivers
Counseling for for older adults in the process of adjusting to changes in housing,
health, employment, finances and relationships
Provide assistance with housing... (Apartments, Assisted Living, Board and Care homes, etc...)
Transitions Program
A structured support group to address issues of loss of a loved one, health problems, lonliness, isolation, depression and decrease in independence
Same day referrals accepted
Follow-up with facilitator
Group meets twice a month
Transitions Program Goals
To manage isolation, loneliness and depression in a healthy manner.
To enable older adults to realize their potential for independent living, and accept changes in their life.
To provide assistance and support through information and referral services.
Case Management
The Social Service Department will make necessary referrals for frail and homebound seniors to
maintain independence. Referrals include: reassurance calls, friendly visits, home delivered meals and home care assistance.
Counseling
Counselor available to provide supportive counseling and therapy to seniors in the process of adjusting to losses
including housing, employment, finances and relationships.
Financial Counseling
First Wednesday of each month at 9:00 am - 12 noon. Call the front desk for an appointment.
Health Insurance Counseling Advocacy Program (HICAP)
Need help making sense of Medicare, HMOs, health insurance, long term care, or billings? Counselors are available to help you
make sense of it all. Call for an appointment.

Legal Counseling with an Attorney
Call for an appointment. An attorney will counsel and advise on legal matters on the 2nd and 4th Thursday of each month between 5pm - 7pm.
Medical Equipment Loan Program
This short-term loan program provides equipment which includes: canes, crutches, walkers and wheelchairs.
