Health Screening
Preventative Health Care For The
Aging (PHCA) is a community
health assessment program
for adults. Those over the age
of 60 can receive basic health
screenings, health maintenance
counseling and follow-up for
clients with special needs and
health problems.
No medical treatment is provided.
If treatment is required, referrals
and coordination of care with
providers is made available.
Screenings include:
Health History
Nutrition History
Blood Pressure
Vision Screening
Hearing Test
Skin Examination
Oral/Dental Examination
Hemoglobin and Blood Sugar Testing
Height/Weight
Urine Screening
Colo-Rectal Screening
Cholesterol Screening
Nutritional Counseling
Medication Counseling and breast self-examination
Registered nurses (R.N.s) who
are certified in public health
conduct the medical screenings
and provide health and nutritional
counseling. Health education
lectures are provided on such
topics as:
Diabetes
Nutrition
Heart disease
Fall prevention
Medication management
